We've all seen how offensive linemen stand around unoccupied and are unfazed as loop blitzers dash between them and another lineman. The key to glitch logic is identifying why the lineman isn't triggered to engage. Presumably there's an area within which the lineman engages, but that's no doubt modified to acknowledge the subtleties that come with handling lineman stunts. If yours is like the one I saw, I suspect that it has a lot to do with the interaction taking place between the offensive tackle and the defensive tackle. If the results seem to vary, you're probably targeting a particular push from the tackle that causes Madden's logic to count the stunting defensive end as the offensive tackle's assignment, and not the interior lineman's.
A related item, since you mention pressure, is that Fearless doesn't seem to work 100% of the time, which is a real bummer. 4 AP is a lot to pay for 'sometimes avoids a catastrophic result'.
