When I started playing MUT with 2015, Muthead was crazy active. Clans, giveaways, contests, pick 'ems, raffles, Staccocakes etc. Now the forums are inaccessible. What exactly happened to this once prestigious website?

Imo after Legend and rocket left,they gave the reigns to a kid RKA371.He wrecked the site and was the reason i left over there as a Mod.

I stopped using it specifically because of one insufferable power tripping mod on there. Never had to deal with someone on that much of a power trip on any other site, even reddit.
As with many things in life, the answer is complex. As a Muthead OG (Evo before that) I can speak to the lore. Many fanatics on here won't want to accept this, but the #1 reason Muthead died was MUT itself. The fine people on that forum were players from the beginning, before the massive microtransactions and monetization of the game. It's had a sharp slide into a casino, and every year it's gotten worse with pack odds and "step offers". What used to be a game where you could keep up as NMS and grind out objectives turned into a whale's game with more point offers than coin. MUT is in a despicable state, and I promise you it is dying on the vine while whales are propping it up as best as they can. It's the worst of all the sports games, surpassing NBA 2K and FAR WORSE than SDS with The Show. Enough on that, if you don't see how awful the game is, I'm sorry that you bow down to the altar of EA.
(2) is when Nick "took over" Muthead, all development ceased on the site. No mini games, no promo write-ups, it literally was just hanging by a thread by forum members who would post updates for the programs. It turned into a vehicle for promoting the live streams, which you all know are trash and only viewed for the drops.
(3) as some others mentioned, some of the mods got over-zealous with their position. This happens on all forums, even here, but you get people who go in on the game and some mod wants to bootlick EA or the game and they'll censor the post. Some of the current mods here (despite their denials otherwise) have ties with Orlando (aka Tiburon) and do the same thing.
TL;DR. Muthead was for the OGs. OGs don't play the game anymore because it sucks in it current state and it's for whales and kids with daddy's wallet.

Essentially what this guy said. I had around 20k~ something thousand posts on the site and was in a whole bunch of clans like MHMA, and a whole bunch of others I can’t remember anymore, and was huge into the mobile game from MM16-20. When they introduced the fake bot run auction house in MM19? I believe, it really killed a lot of the mobile community on MH as it made all MHC (or whatever they were called) trades in game for coins useless. And then post overdrive they turned it into a copy and paste mobile hero game with no auction house/real gameplay outside of drive based game modes and that completely destroyed all or most discussion around the game, though ironically the mobile part of the forums on muthead that are still somewhat used by a couple people and thus moderated. Also, the removal of clans, raffles, MHC and its value/minigames to get MHC, and the ogs leaving the game and fourm, contributed to its current spot
I bailed when they wouldn't let me update the AH prices, only a select few with x number of posts could. Luckily, mut.gg showed up then.

1 - MHC was killed off by EA
2 - unsolvable spam issue
3 - everyone who made muthead left for MUT.gg

I agree nick was the worse,had no clue on what he was doing.His decision making no doubt accelerated the demise of Muthead,
