1. No, it's not a new meta defense cover 3 zone is one of the oldest and most favorite zone defense for most madden players for a long time, but lately with the abundance of Deep Zone KO players with some discounted ones, everyone runs 2 DZKO and an In Zone KO with maybe a flat zone KO or 2 and lurk artists over the middle. Usually out of this defense they'll press and try to take away deep corners with the outside CBs with KOs while taking away flat routes. Some run mable coverage on one side or both.

2. Cover 3's main weaknesses are in the seams, between the 2 middle zones and the deep middle blue, and towards the sideline if you can create a flood concept. I can't teach you how to run an offense but basically you have to either confuse their user over the middle with a high-low concept with a vertical route somewhere on the field to make their DB's drop back. Flip your plays or motion too if you can
