I’m grinding ranked this season and I’m already at Champion III.
My question is:
If I get into the top 100, do I have to stay there to receive a top 100 reward or can I fall out and still receive the award?

You have to stay there. If you check leaderboards during the last week you'll see alot of top 100 players fall off or move up indicating they continued to play.

Like Ramfans said, you gotta stay in the top 100. For reference (someone correct me if wrong), I believe the top 100 was above 6000 points last month. It's a grind and you have to play an insane amount of games even if you are really good and win 3/4 of your games.
