Is there a list of players who get multiple chem slots for theme teams such as combine 93's?

Only the 4 Super Bowl cards. Tom, Von, Ty and Marcus. Along with combine and team captains.

Are there some cards where there is one slot but 5 times team chem? If so, how do you know it is working and actually giving 5X and not just 1?

go to player upgrade. Go down to team chemistry. When you click on that, it will tell you how many total players there are for each team chemistry.

Believe its just the combine and team captains as of now, some team builders that have played for multiple teams as well. Like baker Mayfield got 2 different team chems

Not many, but yes a couple Team Diamonds get x3 team chems (in a single bucket) and the new, fully upgraded Free Agency Welcome Pack players get the x5 team chems... each time you upgrade, that bucket should count up, until you reach max tier, at which point you can apply the x5, for example: https://www.mut.gg/players/21560-justin-fields/25-38021560/

93 OVR Combine cards are special, in the sense that you can apply two entirely separate / split team chems, but that is not typical -- at least not outside of the various Team Captains options. Hope that helps.
